+100% accuracy should only be possible with explosives, because you can take out more than 1 person with a single blow.
But sometimes a small error occurs when you are reanimated(when holding a knife), if you press "fire" at the same time as the revive, you make a kill. It doesn't happen all the time but it does sometimes.
